Care Domains: Structure for Personal, Systematic Support

CareBuilder organises care through clear care domains, based on the Supports Intensity Scale (SIS). These domains structure all daily, weekly, and monthly activities and form the building blocks of support. This ensures that every relevant area of life is included within the care planning.

The domains align with Positive Health and are directly integrated into the CareBuilder platform and the e-Dashboard. The following domains form the basis:

  • Living & Learning: Activities at home, learning, and lifelong education.

  • Participation: Community activities, employment, and social contacts.

  • Wellbeing & Safety: Health, safety, and advocacy.

  • Specialist Care: Medical and behavioural support.

  • Supplemental: Wellbeing and Sleep.

From Domain to
Care Package

For each domain, we compile care packages with specific activities, fully tailored to the client profile. Within the ‘Home Activities’ domain, we set up packages for:

  • ADL & personal care

  • Household skills

  • Daily structure and planning

​

CareBuilder builds these packages from practical actions that directly increase independence, safety, and quality of life.
