Care pathways form an essential foundation for organising appropriate, person centred care. Within the CareBuilder platform, we distinguish three care pathways:
Mild Intellectual Disability (MID)
This care pathway is designed for people with an IQ between 50 and 85 and limited social independence. With the right guidance, many are able to function independently. Within this pathway, we distinguish between MID High, MID Low and MID Older Adults, allowing support to be tailored to the individual’s level and life stage.


Severe Intellectual Disability (SID)
We also provide a care pathway for people with an IQ between 35 and 49 who require intensive support in daily life, for example with communication, personal care and safety. Within this pathway, we differentiate between SID High, SID Low and SID Older Adults to closely match individual support needs.
Severe Multiple Disabilities (SMD)
For people with both a severe intellectual disability and a physical disability, often with an IQ below 50, the SMD care pathway is available. These individuals are fully dependent on support and usually communicate non-verbally. This pathway also includes people with psychogeriatric conditions (PG) and acquired brain injury (ABI).
