Through a tailor-made touchscreen home hub, clients communicate with care professionals, informal carers, family and friends. The home hub has been introduced with great success also for clients who have little or no experience with the Internet or digital tools and with clients who have psychological and physical limitations. Reminders, information, video consultations, advice and client support are delivered via the home hub. By integrating smart devices and apps on the platform, we have added a series of functions on prevention and long-term care (eHealth), safety, smart home, information, digital and social inclusion, entertainment and well-being. These are used purposefully depending on the situation.
Not only clients but also caregivers need support from colleagues, other specialists, informal caregivers and family. Moreover, it is important to coordinate integrated care with care providers and with clients – both in planned and ad hoc care. CareBuilder’s mobile app and browser-based portals empower them to deliver individualised care and maximise the time they have to give patients the personal they can be proud of.
The care sector is under continuous pressure to reduce costs and improve care outcomes for the elderly, patients with chronic conditions and those undergoing revalidation and therapy. There is a clear need and opportunity to focus on better prevention and drive therapy compliance, to avoid lost revenues across the care value chain and increased costs of complications. At a time when the elderly population is growing to unprecedented proportions, care organizations are already struggling with systemic shortages of qualified staff and with the changing role of the family in modern day care. 80% of hospitals and long-term care organisations in 2018 have job vacancies they cannot fill. 40% of elderly living in old age homes thinks the care they receive is too hasty. 51% of nurses and caregivers for the elderly find that there is too little time to deliver the care that is actually needed. As much as 61% finds there is no time for personal care. But beyond the growth of the elderly population, greater life expectancy also drives the nature of the care required by the elderly, which is becoming increasingly complex – often requiring different medical specialisms and better integration.
At the same time, patients and the elderly want to maintain a sense of independence, individuality and purpose in their own lives as long as possible. Several studies have clearly shown that patients revalidate better in the comfort of their own homes whenever possible, provided they can count on the right support at the right time. A weekly 30 minute visit from the physiotherapist is of limited use for someone undergoing revalidation following a stroke, if the patient does not follow her exercise routine the rest of the week. It is generally more effective to deliver therapy frequently, and it is more efficient to do this remotely and/or with the help of push content. Similarly patients at home benefit from prompt help when the need arises, rather than having to wait for the next planned appointment – e.g. advice on taking medicines and their side effects, performing a stoma or measuring blood pressure and glucose levels. In the age of tablets, wearable devices, smart homes and the Internet of Things, plug-and-play applications can be easily adapted to the individual and complex needs of the elderly and of patients requiring long-term care or revalidating from hospital interventions.
But it is not only patients who can benefit. Caregivers are under enormous pressure to deliver high quality care, ever more efficiently. Whether professional or informal, caregivers often feel left to their own devices, lacking support and feeling unsafe. When there is no time to deliver patients the care and personal attention they need, job satisfaction suffers greatly suffers and often results in increased stress, burnout and high staff turnover. That a large part of caregivers’ time is spent on non-core activities such as admin, time recording and travelling to patients’ locations, does not help. Here too, technology can play an important role in helping caregivers feel connected, safe and supported, always.
